End-of-Life Care

A person-centred and integrated health service that pays special attention to the needs and preferences of end-of-life patients at home

This service is focused on providing palliative care to patients in their end-of-life passage such as frequent home medical and nursing visits (depending on the medical condition of the patient), after office hours service and psycho-social support to patients and their caregivers.
