Home Medical & Nursing

A home medical and nursing service offering primary healthcare to the frail and needy seniors

The service involves sending a team of professionals, including a doctor, nurse and medical social worker to assess a patient’s overall condition before prescribing a comprehensive care plan that considers the medical, social, functional and psychological needs of the patient. The care plan is then rolled out by an assigned nurse and doctor, together with the caregiver and family.
