Job Description
- Answer enquiries from incoming calls/emails, provide information and referral services.
- Maintain, update and monitor referrals and admission data for statistical purpose.
- Support needs and intake assessment such as monitor means testing, conduct financial assessments, conducting home visits as required.
- Assist MSW to conduct and prepare social report to apply for financial aids and apply necessary financial schemes to help clients.
- Prepare comprehensive case notes and maintain all relevant documents accurately.
- Provide general, administration support and perform any other duties as assigned.
- Support in building rapport and professional relationships with community partners and stakeholders.
- Support in the development and implementation of programmes.
- Support in data collection for research projects.
Requirements
- Professional qualifications including a Diploma in Social Work / Community Service / GCE ‘A’ / GCE ‘O’ Levels in relevant field
- Minimum 2 years of work experience in acute and/or community healthcare setting
- Positive attitude, people-oriented with pleasant personality
- Good interpersonal and communication skills
- Motivated, resourceful, and mature team-player, who enjoys working with the elderly
- Proficient in IT, able to use email, calendar, shared drives as well as common software and smart phone applications
