Job Description

• Provides home nursing care.
• Makes the initial home visit and performs a biopsychosocial and environmental assessment and conducts subsequent visits.
• Develops a nursing care plan and implements nursing interventions as outlined in the care plan.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Provides nursing services requiring substantial and specialized nursing skill.
• Provides patient and caregiver education.
• Prepares clinical documentation.
• Co-ordinates with other team members and updates them of changes in the patient’s status and needs.
• Evaluates the patient’s response to care interventions on a regular basis and recommends revision of care plans as necessary.
• Functions as the primary care manager for a given patient, where appropriate.
• Assists the nurse-in-charge with stock inventory.

Requirements

• Registered Nurse with Singapore Nursing Board
• Basic computer skills for electronic health record documentation
• Good communication & interpersonal skills

Job Description

• Provides home personal care services such as assistance with activities of living (eg. showering, dressing, feeding, toileting, transferring, moving around), eldersitting, medication, mind-stimulating exercises, and simple exercises.
• Works closely with the clinical team to ensure that the patient is well taken care of.
• Assists nurse-in-charge to perform biopsychosocial and environmental assessment on the initial visit.
• Co-ordinates with other team members and updates them of changes in the patient’s status and needs.
• Able to take basic vital signs. Eg. Temperature, blood pressure and respiration rate.
• Assists the nurse-in-charge with stock inventory.

Requirements

• ITE Skills Certificate in Healthcare (Home Care) or WSQ Higher / Advanced Certificate in Health Care Support (Nursing Care)
• Basic command of English, Mandarin and local dialects would be an added advantage
• Basic computer literacy
• Good team player

Job Description

• Answer enquiries from incoming calls/emails, provide information and referral services.
• Maintain, update and monitor referrals and admission data for statistical purpose.
• Support needs and intake assessment such as monitor means testing, conduct financial assessments, conducting home visits as required.
• Assist MSW to conduct and prepare social report to apply for financial aids and apply necessary financial schemes to help clients.
• Prepare comprehensive case notes and maintain all relevant documents accurately.
• Provide general, administration support and perform any other duties as assigned.
• Support in building rapport and professional relationships with community partners and stakeholders.
• Support in the development and implementation of programmes.
• Support in data collection for research projects.

Requirements

• Professional qualifications including a Diploma in Social Work / Community Service / GCE ‘A’ / GCE ‘O’ Levels in relevant field
• Minimum 2 years of work experience in acute and/or community healthcare setting
• Positive attitude, people-oriented with pleasant personality
• Good interpersonal and communication skills
• Motivated, resourceful, and mature team-player, who enjoys working with the elderly
• Proficient in IT, able to use email, calendar, shared drives as well as common software and smart phone applications
